Please book appointment for an eye examination that requires completion of DMV form. It is important to note that we do not facilitate DOT forms. Additionally, if glasses are necessary, please be aware that the form will not be considered complete until the glasses have been received.

Please book appointment for finalizing contact lens prescriptions to be made exclusively by individuals who have previously undergone a contact lens fitting examination with us and now require their prescription to be finalized in-office.
